Note for launch: this is the web version of Katie's printed worksheet. Three deliberate differences: (1) passport numbers and dates of birth are not collected — name + DOB + passport number in one emailed form is an identity-theft package, so those belong in a secure channel at booking time. (2) Section 8 of the paper form (intake date, quote sent, final payment, documents sent) is Katie's internal tracking, not something a client fills in, so it's omitted. (3) The 8-row traveler table became a single names box, which is far less painful on a phone.
